About the role
Full time Social Worker - Hospice Albany and Rensselaer Counties. The Community Hospice has a full time opportunity for a social worker to join our hospice care team caring for patients in primarily the Albany/Rensselaer County area as part of our Interdisciplinary Team in homes and nursing homes. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. The Community Hospice provides a thorough orientation and a supportive work environment.
Responsibilities
- Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care
- Provide specialized skills and mental health therapy for clients
- Identify barriers which impact optimal patient functioning
- Ensure patients receive appropriate resources in most appropriate setting
Requirements
- NYS SW Licensure
- Valid NYS Drivers license
- Minimum 2-3 years experience ideally caring for seniors or those with advance illness

Pay
Pay Range: $30.00 - $43.08. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
Schedule
Position is Full time Monday‑Friday 8a‑4:30p.
