Full‑time Social Worker – Hospice for Columbia and Greene Counties. The Community Hospice seeks a social worker to join its interdisciplinary hospice care team, providing care to patients primarily in Columbia County, both in homes and nursing homes. The role focuses on responding to the emotional and psychological needs of patients, families, and staff.

Responsibilities

• Diagnose and treat psychological, behavioral, and emotional disorders as they relate to end‑of‑life care.
• Provide specialized mental‑health therapy and support for clients.
• Identify barriers that impact optimal patient functioning.
• Ensure patients receive appropriate resources in the most suitable setting.

Requirements

• New York State Social Worker licensure.
• Valid New York State driver’s license.
• Minimum 2–3 years of experience, ideally caring for seniors or individuals with advanced illness.

Pay

Pay Range: $30.00 – $43.08 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The range may vary based on location.

Schedule

Full‑time, Monday – Friday, 8:00 a.m. – 4:30 p.m.
