Full Time Physical Therapist (PT) for Home Health - Longview, TX

About the role

Highland Park Senior Care is hiring an experienced and compassionate Full Time Physical Therapist (PT) to provide care in patients' homes in Longview, TX 75601. The ideal candidate will conduct patient assessments, develop the Plan of Care, and treat patients to relieve pain and restore function.

Responsibilities

• Conducts patient assessments and evaluates functional ability
• Aids in the development of the Plan of Care
• Directs and aids patients in exercise, muscle re-education, and training
• Treats patients to relieve pain and maintain function
• Uses various therapeutic modalities and observes patient responses
• Instructs patients and significant others on proper use of medical devices
• Visits patients according to the Plan of Treatment and documents progress
• Prepares discharge summaries and condition summaries

Requirements

• Current state license as a Physical Therapist
• Graduate of an American Physical Therapy Association approved program
• At least 1 year of experience in home health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Excellent time management and organizational skills
• Valid driver's license and reliable transportation
