Full Time Physical Therapist (PT) for Home Health - Longview, TX

About the role

Frontpoint Health is a fast-growing home health agency dedicated to providing personalized, high-quality care. We are currently hiring an experienced and compassionate Physical Therapist (PT) to join our team and help patients recover safely and comfortably in their homes.

Responsibilities

• Counsels and educates patients, significant others, and staff on the care and use of wheelchairs, prosthetics, and orthopedic devices.
• Treats patients to relieve pain, restore function, and maintain maximum functioning through various therapeutic methods including exercise, massage, heat, water, light, and electricity.
• Applies ultraviolet and infrared lamps, diathermy, ultrasound, whirlpool, contract baths, and moist packs as needed.
• Observes, records, and reports the patient's response to treatment and changes in their condition to the physician and supervising therapist.
• Instructs patients and significant others in areas of physical therapy in which they can participate and assist the patient.
• Visits patients according to the Plan of Treatment, completes a progress note for each visit, and submits progress notes timely.
• Prepares a written discharge summary and a written summary report on a patient's condition at least every 60 days.

Requirements

• Current state license as a Physical Therapist.
• Graduate of an American Physical Therapy Association approved physical therapy program.
• Minimum of 1 year of experience in home health.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Excellent time management and organizational skills.
• Valid driver's license and reliable transportation.

Schedule

Day shift Monday to Friday. Weekend availability (optional).
