About the Role
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform in-home health assessments for Medicare Advantage and other plan members.
Responsibilities
- Perform in-home assessments and physical exams
- Document diagnoses and collaborate with primary care providers
- Identify urgent or emergent issues and intervene appropriately
- Conduct comprehensive evaluations, including reviewing medical history, medication reconciliation, vital signs, and screenings
- Communicate findings to primary care providers and identify diagnoses for care management
- Educate patients on disease management and medication adherence
- Address social determinants of health and coordinate referrals
Requirements
- Active, unrestricted licensure and certification (ANCC, AANP, or NCCPA)
- Valid driver’s license and reliable transportation
- Ability to navigate home environments and perform physical assessments
Preferred Skills
- 1+ years of clinical experience
- Proficiency with EMRs
- Strong communication skills, particularly with geriatric or Medicare populations

Pay
Compensation ranges from $125,000 to $175,000 plus benefits and incentives.
