Full Time - Nurse Practitioner or Physician Assistant

About the role

We are seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home assessments aimed at improving patient health outcomes.

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, physical exams, screenings, and point-of-care testing.
• Identify diagnoses, document findings, and collaborate with primary care providers to address care gaps.
• Recognize urgent situations and provide patient education on disease management and social determinants of health.
• Refer patients to appropriate resources and ensure follow-up care.

Requirements

• Active, unrestricted licensure in the practicing state and relevant certification (ANCC, AANP, or NCCPA).
• Valid driver’s license and reliable transportation for home visits.
• Ability to navigate diverse home environments and perform physical assessments.

Qualifications

Preferred qualifications include:

• 1+ years of clinical experience.
• Proficiency with EMRs.
• Strong communication skills, particularly with geriatric or Medicare populations.

Schedule

Flexible scheduling available between 8:00 AM and 8:00 PM, 7 days a week. No on-call, weekend, or holiday shifts required.

Pay

Competitive compensation ranging from $125,000 to $175,000 per year, plus benefits.
