About the role
Join a team providing comprehensive in-home health assessments to improve patient care and outcomes.
Responsibilities
- Conduct adult and geriatric health assessments, including medical history, diagnosis, treatment, and health education.
- Administer point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, social workers, caregivers, and medical personnel.
- Coordinate follow-up care, referrals, and documentation per protocols.
- Provide services at members’ residences, including homes and Skilled Nursing Facilities.
Requirements
- Master’s Degree or equivalent experience with NP/PA licensure and prescriptive authority.
- Board certification in Adult, Family, or Geriatric primary care or acute care.
- Current BLS, ACLS, or CPR certification.
- At least 1 year of NP or PA experience.
- Ability to travel extensively, including overnight stays, with a valid driver’s license.
- Strong communication and computer skills, and familiarity with medical record software.
