Full-Time Nurse Navigator

Role: Nurse Navigator RN responsible for guiding patients through complex care pathways in a dynamic, mission-driven setting. Key Responsibilities Coordinate appointments, referrals, and services across multidisciplinary teams to ensure seamless patient transitions.Perform clinical assessments and provide education to patients and their families.Address and resolve barriers to care to facilitate timely treatment.Document all interactions accurately in EMRs while maintaining strict safety and quality standards.Collaborate with EMS and hospital partners to deliver compassionate, high-quality care.Enhance specialized clinical skills through ongoing professional engagement. Qualifications & Skills Current RN license with clinical experience relevant to patient navigation and care coordination.Strong communication and interpersonal skills to educate patients and collaborate with teams.Ability to assess patient needs, address barriers, and ensure safe care transitions.Proficiency in documentation, EMRs, and adherence to safety and quality protocols.
