About the role
Nurse Navigator RN responsible for guiding patients through complex care pathways in a dynamic environment.
Responsibilities
- Coordinate appointments, referrals, and services across multidisciplinary teams to ensure safe and timely patient transitions.
- Perform clinical assessments, provide patient and family education, and address barriers to care.
- Document all activities accurately in Electronic Medical Records (EMRs) while maintaining safety and quality standards.
- Collaborate with EMS and hospital partners to deliver compassionate, life-impacting care.
- Contribute to the advancement of specialized clinical skills through ongoing engagement in patient care activities.
Qualifications & Skills
- Registered Nurse (RN) licensure
- Strong clinical assessment skills
- Excellent communication and collaboration abilities
- Patient-centered approach to care
