Full-Time Nurse Navigator

Role: Nurse Navigator RN, responsible for guiding patients through complex care pathways in a dynamic environment. Key Responsibilities: Coordinate appointments, referrals, and services across multidisciplinary teams to ensure safe and timely patient transitions.Perform clinical assessments, provide patient and family education, and address barriers to care.Document all activities accurately in Electronic Medical Records (EMRs) while maintaining safety and quality standards.Collaborate with EMS and hospital partners to deliver compassionate, life-impacting care.Contribute to the advancement of specialized clinical skills through ongoing engagement in patient care activities. Qualifications & Skills: Registered Nurse (RN) licensure, strong clinical assessment skills, excellent communication and collaboration abilities, and a patient-centered approach to care.
