Full-Time LCSW Case Manager

Responsibilities

• Conduct psychosocial assessments
• Develop individualized care plans
• Provide ongoing case management for diverse patient populations
• Coordinate interdisciplinary care teams
• Facilitate referrals to community resources
• Support discharge planning
• Deliver brief therapeutic interventions
• Track outcomes
• Ensure regulatory compliance through accurate documentation and data management

Qualifications & Skills

• Licensed Clinical Social Worker (LCSW)
• Strong clinical assessment skills
• Excellent communication and collaboration abilities
• Knowledge of healthcare regulations
• Ability to work effectively with physicians, nurses, and other healthcare professionals to enhance patient experience and quality of care
