Case Manager - LSW or LCSW FT
About the role
Under the direction and supervision of the LCSW Field Instructor and/or designee(s), the Social Worker provides case management and social work services to patients and families within assigned areas to meet identified psychological needs, enhance problem solving and coping capacities, and facilitate timely discharge by arranging post-discharge services. The Social Worker conducts psychosocial assessments, supportive counseling, crisis intervention, and financial planning, collaborating with all members of the healthcare team to ensure clear communication and care planning. The role works with the multidisciplinary team to ensure safe transitions of care and effective utilization of resources.
Responsibilities
- Participate in patient progression throughout the continuum to achieve desired outcomes and organizational goals while promoting continuity of care, collaborative practice, and appropriate utilization of resources.
- Collaborate with all members of the interdisciplinary team, external customers, and payor sources to facilitate care coordination and delivery.
- Monitor patient progress, intervening as directed to ensure the plan of care is patient-focused, efficient, cost-effective, and maximizes clinical outcomes.
- Identify and assist in resolving delays and obstacles to discharge.
- Meet with patients/families to assess needs and develop a patient-centered discharge plan, collaborating with the interdisciplinary team in all phases of the discharge planning process.
- Participate in the referral process for home health, Hospice, durable medical equipment and supplies, nursing home placement, and Swing Bed placement.
- Identify at-risk populations using approved screening tools and follow established reporting procedures.
- Monitor length of stay and ancillary resource use, taking action to achieve continuous improvement in both areas.
- Seek consultation from appropriate disciplines/departments to expedite care and facilitate timely discharge.
- Support the collection of resource utilization data and clinical outcomes for performance and outcome indicators.
- Demonstrate professional communication skills in written, verbal, and nonverbal interactions.
- Utilize critical thinking and innovative approaches to promote constructive problem-solving.
- Perform other duties as assigned.
Schedule
Full-time position, Monday – Friday 8 am – 4:30 pm. Occasional weekend coverage required.

- Use a keyboard daily for data entry and documentation.
- Visual acuity to observe patients, obtain information, and use documentation.
- Auditory acuity to hear patients, families, and staff for communication purposes.
Reporting Relationship
Reports to the Director of Quality and Case Management, indirectly to the Executive Director of Quality Assurance.
