Full-Time Healthcare Provider

About the role

The provider conducts comprehensive health assessments in members' homes or facilities, focusing on medical history, medication adherence, social factors, and overall health to improve care quality and identify undiagnosed conditions.

Responsibilities

• Perform adult/geriatric assessments, including diagnosis, treatment, health education, and documentation.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, family members, and other healthcare professionals.
• Coordinate follow-up care and patient education with primary care providers.
• Work primarily in members' residences, including homes and skilled nursing facilities.

Qualifications

• Master's Degree or equivalent experience with NP/PA licensure and prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care.
• Current BLS, ACLS, or CPR certification.
• At least 1 year of NP or PA experience.
• Ability to travel extensively, including overnight stays, across states.
• Valid driver’s license and insurance coverage.
