About the Role
Provides patient care coordination, ensuring continuous assessment and implementation of interdisciplinary care plans in compliance with regulations.
Responsibilities
- Perform services based on physician orders and care plans, supporting optimal patient outcomes.
- Maintain thorough documentation and ensure adherence to federal and state guidelines.
Qualifications & Skills
- Must be a graduate of an approved clinical school, licensed in the practice state.
- At least one year of professional experience.
- Basic technology skills and the ability to operate mobile devices.
- Valid state driver license, dependable transportation, automobile insurance, and CPR certification.
- Must be able to drive safely in all weather conditions.
Preferred: Prior experience in Medicare home health or hospice. Leadership may review and approve candidates with limited experience.
