Case Manager / RN responsible for coordinating safe and efficient patient care across diverse populations.
Responsibilities
- Perform comprehensive patient assessments
- Develop and manage individualized care plans
- Lead discharge planning processes
- Ensure appropriate utilization of healthcare services
- Collaborate with physicians, nurses, social workers, and payors to optimize outcomes
- Support quality improvement initiatives, patient satisfaction, and reduction of readmissions
Qualifications & Skills
- Registered Nurse licensure with strong clinical assessment skills
- Excellent communication and collaboration abilities
- Knowledge of care coordination and discharge planning processes
- Commitment to compassionate, safe, and patient-centered care
