About the Role

Coordinate safe and efficient patient care across the continuum in a compassionate, patient-centered, and community-focused environment.

Responsibilities

• Conduct comprehensive clinical assessments and develop individualized care plans.
• Lead discharge planning to facilitate smooth patient transitions.
• Collaborate with physicians, social workers, payers, and community resources to ensure appropriate utilization and remove barriers.
• Educate patients and families on care plans and support understanding.
• Document in the EHR and track patient outcomes.

Requirements

• Registered Nurse (RN) licensure.
• Strong clinical assessment and care planning skills.
• Excellent communication and collaboration abilities.
• Knowledge of healthcare utilization, discharge processes, and community resources.
• Ability to educate and support patients and families effectively.
• Proficient in electronic health records (EHR) documentation.
• Focused on quality, cost-effective care and patient outcomes.
