FT RN Case Manager (Dorchester/Wicomico)

Responsibilities

  • Develop, implement, and evaluate a plan of patient care utilizing ANA Standards and the Nursing Process.
  • Utilize equipment and technology skillfully and demonstrate technical competence in nursing procedures.
  • Oversee and/or participate in safe medication handling and practices.
  • Educate patients and families on significant self-care and end-of-life issues, all hospice services, patient liability for payment, and ensure consent and payer forms are completed.
  • Act as a patient advocate, providing support, information, and direct care.
  • Triage phone calls to identify patient/family needs and notify Admission Services of referrals.
  • Oversee and delegate assignments of CMTs, LPNs, Hospice Aides, students, and volunteers on shift.
  • Participate in weekend, holiday, on-call, and/or evening/night rotations.
  • Maintain appropriate licenses and adhere to continuing education obligations.
  • Cover different areas of hospice care to assure organizational needs are met.

Inpatient Unit RN

  • Provide medical care to hospice patients in an inpatient setting.
  • Complete assessments, administer medications, and manage the plan of care for patients during their shift.
  • Coordinate discharge planning with family and medical provider.

Extended Services RN

  • Provide medical care to patients in their home environment based on triaged needs.
  • Utilize nursing assessment and symptom management skills to assist with urgent after-hours needs.
  • Educate patient/caregiver on plan of care in coordination with the on-call medical provider.

RN Case Manager

  • Provide medical care to a manageable caseload of hospice patients in their home settings.
  • Develop a plan of care alongside an Interdisciplinary Team in line with the patient/family needs and goals.
  • Coordinate all aspects of home hospice care including symptom management, spiritual, psychosocial, financial, and bereavement needs.
  • Work closely with the Interdisciplinary Team to assure patient/family goals are met.

Requirements

  • Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware, or other compact state.
  • BSN from an accredited college preferred.
  • CHPN certification expected within two (2) years of employment.
  • Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA, FMLA, OSHA, and other federal, state, and local laws.
  • Must be able to lift and carry up to fifty (50) pounds with or without the use of assistive devices.
  • Understand and comply with Coastal Hospice’s mission, values, vision, policies, and Employee Handbook.
