Responsibilities

  • Deliver onsite primary care services to older adults in Senior Living Communities
  • Develop and execute detailed person-centered care plans on all primary care patients
  • Work with the Medical Director to identify and develop policies addressing issues affecting day-to-day practice, culture, or clinical guidelines, including for emerging diseases
  • Conduct monthly Chronic Care Management (CCM) visits and care reviews; sign off on CCM documentation
  • Lead regular huddles and provide leadership to the clinical team
  • Collaborate with senior living management and care teams to implement Pine Park care plans and enhance patient outcomes

Qualifications

  • Physician Assistant with experience in geriatrics
  • Comfortable working in a fast-paced, changing environment
  • Willingness to provide onsite clinical care across the Central Phoenix/Tempe region
  • A strong orientation towards collaboration and working closely with a multidisciplinary team
  • A strong written and verbal communicator
  • Ability to work with tech-enabled care delivery tools to improve patient care and team function
  • A flexible work schedule with travel within the Phoenix area
