FT Nurse Practitioner - San Diego

About the role

Pine Park Health is a value-based primary care practice dedicated to transforming senior healthcare. We offer prevention, screening, chronic condition management, lab work, and diagnostic testing directly to seniors in their homes, supported by weekly community visits and a collaborative care model.

Responsibilities

• Provide onsite primary care services to older adults living in Senior Living Communities
• Develop and execute detailed person-centered care plans on all primary care patients
• Work with Medical Director to identify and develop policies addressing issues affecting day-to-day practice, culture, or clinical guidelines, including for emerging diseases
• Run regular huddles and provide leadership to the clinical team
• Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes

Qualifications

• Nurse Practitioner - preferably with geriatrics experience
• Comfortable with a fast-paced and changing environment
• Willingness to provide onsite clinical across the Northern San Diego region
• Desire to work with complex care patients and support the building of a holistic care model
• Strong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical team
• Strong written and verbal communicator
• Ability to work with tech-enabled care delivery tools (e.g., project management, 'checklist' management, remote monitoring, etc.) used together to improve patient care and team function
• Flexible work schedule with travel within the Northern San Diego area

Pay

The base salary range for this role is $135,000 to $150,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate.
