About the Role
As the founding Head of Care Delivery, you will serve as the lead navigator for seniors and families while building the operational foundation for the care delivery function. You will transition from individual execution to scaling a team, defining protocols, and establishing the system of record for aging care management.
This is a remarkable opportunity to shape a generational care model from day one as the very first hire in this critical function. You’ll join a well-funded, pre-launch startup led by experienced health-tech founders with a multi-year runway and Medicare coverage. The role combines AI and human advocacy to solve one of the most fragmented challenges in modern healthcare.
Responsibilities
- Directly manage the first cohort of patients, coordinating complex care needs and closing care gaps for seniors and families.
- Build the navigation operating system, including protocols, escalation rules, quality assurance standards, and the care delivery playbook.
- Hire, train, and lead the future navigation team while owning core metrics like enrollment and patient NPS.
Requirements
- Proven experience navigating complex healthcare or eldercare systems, preferably with a clinical or social work background (RN, LCSW, SW).
- Entrepreneurial mindset with a track record of building programs or processes from scratch in high-stakes, ambiguous environments.
- Exceptional empathy and advocacy skills combined with the operational discipline to build scalable systems.
