Intercept Health is a family‑owned agency serving mental and behavioral health, substance use, and foster care needs throughout Virginia. Founded in 1996 as a single group home for boys, the organization has grown to meet the service needs of families and communities across the Commonwealth. The full‑time Treatment Foster Care Family Consultant provides casework services to children and families and reports to the TFC Program Supervisor.

Education & Experience

• A baccalaureate degree in any field plus two years’ experience providing casework services to children and families.
• A baccalaureate degree in social work or a related field (sociology, psychology, special education, counseling) and one year of casework experience with children and families.
• A doctorate or master’s degree in social work (CSWE‑accredited) or a related field with a student placement in casework services to children and families, or one year of relevant experience.

Responsibilities

• Interview children and families.
• Conduct and write home studies.
• Develop individualized treatment and service plans.
• Counsel children and families in preparation for placement or discharge.
• Supervise children in resource, foster, adoptive homes, and independent living arrangements.
• Prepare and maintain case records.
• Coordinate services to minimize fragmentation of care, reduce barriers, and link children with appropriate medical, social, educational, and other services.
• Periodically assess children’s psychosocial, nutritional, medical, and educational needs.
• Assist with referrals, arrange appropriate services, and ensure continuity of care for children in treatment foster care.
• Link children to services and supports specified in their individualized treatment plans.
• Coordinate service planning with other agencies and providers, arranging medical, remedial, and dental services as needed.
• Monitor ongoing progress, ensure services are delivered, and continually evaluate each child’s plan of care.
• Collaborate with family assessment and planning teams and other parties on reviews and coordination of services.
• Educate and counsel children, fostering supportive relationships that promote the service plan.
