About the role
Provides supportive counseling, ADL training, crisis intervention and restorative services (when applicable) to people with psychiatric disabilities in residential programs in compliance with relevant regulations.
Responsibilities
- Manage a rotating schedule at all 24 hour supervised sites, location of coverage will be changing and determined by site need and management.
- Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services.
- Maintain a clean, safe and homelike environment by monitoring, training and assisting residents in household maintenance activities.
- Provide coverage at all residential sites as needed.
- Train and assist residents in daily activities including food purchases, recreational activities, and meal planning and personal care activities.
- Develop and support meaningful service plan goals for residents and provide necessary supports to achieve these goals.
- Conduct fire safety training and ensure fire drills are conducted in accordance with regulatory guidelines.
- Participate in Medicaid billable activities and ensure all documentation is current and compliant with regulatory guidelines.
- Document interactions with residents according to agency policies and standards.
- Meet weekly with assigned primary residents for counseling and goal review.
- Maintain collateral contacts with outside service providers and participate in case conferences as scheduled.
- Aid residents in obtaining and maintaining financial entitlements.
- Monitor medications as required.
- Provide 24-hour supervision to residents in congregate facilities.
- Provide transportation as needed using personal vehicle when agency vehicle is unavailable.
- Promptly report all emergencies, incidents, maintenance and safety issues to senior counselor or supervisor.
- Attend team meetings as scheduled.
- Complete admission documentation post-intake for new clients.
- Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress.
- Participate in Quality Improvement (QI) training during onboarding and attend QI refresher training as needed.
- Cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
- Adhere to all safety protocols and procedures to ensure a safe working environment for all employees.
- Show awareness of health and safety protocols to maintain a safe work environment.
Qualifications
- High School Diploma required
- At least 5 years' experience preferred in a mental health related field
