• Provide high-quality home-based rehabilitation services across Manhattan.

• Educate patients and caregivers on treatment plans and safety.
• Coordinate closely with interdisciplinary care teams.
• Document all care activities accurately in an EMR system.

Requirements

• Licensed Physical Therapist with strong clinical skills.
• Ability to perform detailed assessments and develop tailored treatment plans.
• Excellent communication and patient education skills.
• Comfortable working independently and collaboratively.
• Proficiency in documentation and EMR systems.
