About the role
Visiting Angels is seeking a Home Care Field Registered Nurse (RN)/Case Manager for in-home visits to their clients throughout Mercer County and some Burlington County clients, as well.
Responsibilities
- Making appointments with Visiting Angels' clients in their Mercer County homes during a regular Visiting Angels' care shift
- Mileage is reimbursed per Company policy
- Create in-home initial client assessments and care plans based on clients' specific care needs
- Conduct follow-up in-home assessments at specified intervals
- Conduct periodic telephone assessments
- Revise care plans as necessary
- Supervise Certified Home Health Aides (CHHAs)
- Perform Orientations/Skills Assessments of CHHAs
Qualifications
- Previous Home Care RN experience (HIGHLY preferred)
- Current New Jersey Registered Nurse (RN) license
- Ideal candidate resides in Mercer County
- Enjoys meeting and talking with older adults
- Prior Registered Nurse (RN) experience with in-home care assessments
- Excellent written and verbal communicator with strong interpersonal skills
- Must be self-motivated, organized, and able to work independently
- Able and willing to travel to clients' homes throughout Mercer and/or Burlington Counties
- Has a valid driver's license and a reliable car, willing to travel to clients' homes
- Adept with digital/computer skills (data entry on a web-based program, email, scanning, texting)
- Has weekday flexibility
