Field Registered Nurse RN Case Manager for Home Care Mercer County

Visiting Angels is seeking a Home Care Field Registered Nurse (RN)/Case Manager for in-home visits to clients throughout Mercer County and ideally some Burlington County clients (optional). This is a non-medical per diem position with no set hours, offering flexible scheduling ideal for an RN who desires significant work flexibility. Most visits occur on weekdays.

Compensation

$70 per visit for initial assessments and $60 per visit for 60-day visits.

Responsibilities

• Schedule and conduct in-home appointments with Visiting Angels' clients during regular care shifts (mileage reimbursed per company policy).
• Create thorough and accurate initial client assessments and personalized care plans based on clients' specific care needs.
• Conduct follow-up in-home assessments at specified intervals.
• Perform periodic telephone assessments.
• Revise care plans as necessary.
• Supervise Certified Home Health Aides (CHHAs).
• Perform orientations and skills assessments of CHHAs.

Requirements

• Current New Jersey Registered Nurse (RN) license.
• Previous home care RN experience highly preferred.
• Ideal candidate resides in Mercer County.
• Enjoys meeting and talking with older adults.
• Prior RN experience with in-home care assessments.
• Excellent written and verbal communication skills with strong interpersonal abilities.
• Self-motivated, organized, and able to work independently.
• Able and willing to travel to clients' homes throughout Mercer and/or Burlington Counties.
• Valid driver's license, reliable car, and willingness to travel.
• Adept with digital/computer skills (data entry on a web-based program, email, scanning, texting).
• Weekday flexibility.
