Job Summary

Field Nurse Supervisor- Atlanta area (must be able to spend two (2) to four (4) nights a month out of town)

Responsibilities

  • Manage all clinical and non-clinical staff responsible for client care
  • Develop and maintain a professional working relationship with each client you are responsible for
  • Collect and ensure that all new client admission paperwork is completed to the satisfaction of the state(s) regulatory requirements for home healthcare agencies
  • Ensure all client files are kept in accordance/compliance with all state(s) regulatory guidelines
  • Ensure that all services comply with company policies & procedures, and that state and federal regulations are followed
  • Create and upload a one page “client cheat sheet” for Scheduling Coordinators to email caregivers prior to them covering a shift with a new client
  • Work closely with the internal clinical support team, provide regular updates re: client care, any concerns, or issues with the client’s you are responsible for
  • Periodically attend sales and marketing events
  • Develop, implement, and update client care plan(s) as needed
  • Perform supervisory visits on all clients in accordance with state(s) regulatory guidelines or as warranted
  • Conduct supervisory visits and performance reviews of clinical & non-clinical staff in accordance with state(s) regulatory guidelines
  • Communicate with both internal & external case managers re: client cases you are responsible for
  • At all times and as soon as possible, communicate any changes to client’s needs or caregiver issues that you learn of to Collage’s internal team
  • Answer or promptly return phone calls and emails from Collage internal staff, case managers, and caregivers regarding client care
  • Train new staff on the particulars of each case you are responsible for
  • Conduct staff in-services for each new client prior to client’s start date
  • Work closely with the Scheduling Coordinators to ensure that all client shifts are covered, assist in monitoring of staff, and prioritize caregiver duties
  • Provide counseling and guidance to clients and/or their families

Qualifications

  • Experience with Traumatic Brain Injury and Spinal Cord Injury is preferred
