Position Summary

We have an exciting opportunity to join our team as a Field Nurse, Home Care - Per Visit **EASTERN LONG ISLAND** (1 Year of Home Care Experience Highly Preferred!!).

Job Responsibilities

  • Completes a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age related, environmental, spiritual, transpersonal and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient.
  • Performs other duties as assigned.
  • Affixes Agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction.
  • Participate in Agency quality assessment performance improvement initiatives.
  • Completes and maintains accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education intervention and evaluation according to Federal, Department of Health and The Joint Commission requirements.
  • Identifies deficits and barriers to effective knowledge understanding of education provided to patients. Employs strategies to optimize patient engagement.
  • Demonstrates caring behaviors toward patients, family and other caregivers.
  • Delivers direct nursing care, ongoing assessment, teaching and training to patients caregivers that preserves and protects patient autonomy, dignity, rights, values, and benefits using evidence based nursing practice.
  • Derives the diagnoses and needs from the assessment data, validates the diagnoses, and collaborates with the patient, family, caregivers and physician to develop an individualized plan care to attain expected quality outcomes.

Minimum Qualifications

  • A Bachelor's degree in Nursing required for employees hired after 9/1/2020 (those who were hired prior to 9/1/2020 are grandfathered).
  • Current and valid New York State Registered Professional Nurse license.
  • Current and valid BLS certification.
  • Knowledge of standards of practice for all areas of patient care.
  • Excellent communication (oral and written), organizational, customer service, and interpersonal skills.
  • Basic computer skills.

Required Licenses

  • Registered Nurse License-NYS

Preferred Qualifications

  • Bachelor's Degree Nursing.
