Registered Nurse (RN) – Home Health Visits | $80–$160 Per Visit | East Honolulu

About the role

As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while working closely with physicians, caregivers, and the interdisciplinary care team to support positive health outcomes.

Responsibilities

• Conduct skilled nursing visits in patients' homes
• Complete comprehensive assessments and reassessments
• Develop and update individualized care plans
• Provide patient and family education
• Perform wound care, medication management, home infusions, disease management and other treatments
• Cook up care coordination with physicians and interdisciplinary team members
• Maintain monitoring of patient progress and communicate changes in condition
• Complete accurate and timely clinical documentation
• Ensure compliance with Medicare home health standards and agency policies

Requirements

• Active Hawaii Registered Nurse (RN) license
• Minimum one year of clinical nursing experience
• Home health experience preferred but not required
• Experience with OASIS SOC assessment preferred
• Strong assessment, clinical judgment, and documentation skills
• Ability to work independently and manage a patient caseload
• Reliable transportation and willingness to travel within the East Honolulu service area
• Strong communication and care coordination skills
