Fee For Service Field Nurse - Manhattan $70-$125 per visit

About The Role

The New Jewish Home has a long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible.

Responsibilities

• Provide in-home assessments and reassessments for private ill clients
• Provide intermittent visits in the client’s home including assessment and home health supervision
• Cook up client’s care with care team and create care plan for client’s care
• Document care in EMR in a timely manner
• Flexible work schedule

Requirements

• A valid NYS RN license
• A minimum of one (1) year home care experience as an RN with a CHHA or LHCSA
• A minimum of one (1) year in an acute care or hospital environment
