About the role

As a Care Coach 1, you will assess and evaluate members' needs to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for their care and well-being. This role involves meeting members in their homes, assisted living facilities, long-term care facilities, and other care settings, with 75-90% local travel required. You will create member-specific care plans, ensure services are provided in the least restrictive setting, and coordinate interventions to meet members' needs while aligning with their personal goals.

We are looking for motivated and dynamic case managers residing in Hillsborough, Hardee, Highlands, or South Polk counties in Florida.

Responsibilities

  • Visit Medicaid members in their homes, assisted living facilities, long-term care facilities, and other care settings.
  • Assess and evaluate members' needs to establish a member-specific care plan.
  • Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well-being.
  • Plan and implement interventions to meet members' needs.
  • Coordinate services and monitor/evaluate the case management plan against members' personal goals.
  • Guide members and families toward appropriate resources for their care.
  • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in the member's care.

Requirements

  • Bachelor's degree in a health and human services field.
  • 2 or more years of related experience.
  • Exceptional verbal/written communication and interpersonal skills.
  • Ability to use a variety of electronic information applications/software programs.
  • Must be passionate about contributing to an organization focused on improving consumer experiences.
  • Valid state driver's license and personal vehicle liability insurance (minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
  • A separate room with a locked door for use as a home office to ensure privacy while working.

This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to undergo TB screening.

This role is part of Humana's driver safety program, requiring adherence to vehicle insurance requirements.

Preferred Qualifications

  • Previous experience with electronic case note documentation and documenting in multiple computer applications/systems.
  • Prior experience with Medicare & Medicaid recipients.
  • Experience with health promotion, coaching, and wellness.
  • Knowledge of community health and social service agencies and additional community resources.

Pay

Starting salary is $60,000–$65,000 per year based on qualifications and experience. The pay range for this full-time (40 hours per week) role is $53,700–$72,600 per year, varying by geographic location and individual qualifications.

Schedule

Scheduled weekly hours: 40.

Additional Information

  • This role may require the use of HireVue, an interviewing technology, as part of the hiring process.
  • Home or hybrid home/office employees must have self-provided internet service with a minimum download speed of 25 Mbps and upload speed of 10 Mbps (wireless, wired cable, or DSL suggested; satellite, cellular, and microwave connections may be used if approved by leadership). Employees in California, Illinois, Montana, or South Dakota will receive a bi-weekly payment for internet expenses.
  • Humana will provide telephone equipment for home or hybrid home/office employees to meet business requirements.
  • Work must be conducted from a dedicated space lacking ongoing interruptions to protect member PHI/HIPAA information.
