Field Care Coordinator- Bilingual Preferred
The Care Coach 1 assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for their care and well-being. This role involves straightforward to moderately complex work assignments and reports to the Regional Care Coach Manager.

Responsibilities
- Visit Medicaid members in their homes, assisted living facilities, long-term care facilities, and other care settings (75-90% local travel).
- Assess and evaluate members' needs and requirements to establish a member-specific care plan.
- Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well-being.
- Plan and implement interventions to meet members' needs.
- Coordinate services and monitor and evaluate the case management plan against the member's personal goals.
- Guide members and families toward resources appropriate for their care.
- Facilitate interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member's care as required by our comprehensive contract.
Requirements
- Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology), or
- Registered Nurse (RN), licensed in the state of Florida, with at least 2 years of relevant experience, or
- Licensed Practical Nurse (LPN), licensed in the state of Florida, with at least 4 years of relevant experience.
- Prior experience with Medicare & Medicaid recipients.
- Minimum 2 years of case management experience.
- Valid state driver's license and proof of personal vehicle liability insurance.
- AHCA Fingerprint required.
- This role is patient-facing and requires Tuberculosis (TB) screening.
Preferred Qualifications
- Prior nursing home diversion or long-term care case management experience.
- Bilingual (English/Spanish).
- Experience with electronic case note documentation and documenting in multiple computer applications/systems.
- Experience working with the geriatric population.
- Experience with health promotion, coaching, and wellness.
- Knowledge of community health and social service agencies and additional community resources.
- 701B Certification.
- Medication application (RFA) experience.
Schedule
- Monday–Friday (8:30 AM–5:00 PM).
- 40 scheduled weekly hours.
Pay
The current hiring range for this role is $60,000–$65,000 per year, based on experience and qualifications. The full compensation range is $53,700–$72,600 per year, varying by geographic location and individual qualifications.
