Responsibilities
- Build long-term, trusting relationships with 15-20 families through regular in-home visits (1+ hour each)
- Conduct needs assessments, screenings (ASQ, ASQ-SE, depression screens, FROG Scale), and eligibility evaluations
- Develop individualized Family Service Plans and support families in setting and achieving goals
- Connect families to community resources, healthcare, housing, childcare, and crisis services
- Deliver research-based education on child development, health, nutrition, and family functioning
- Maintain accurate case notes, contact logs, and database records per agency and funding requirements
- Represent the program at community outreach events and agency collaboration meetings
- Participate in weekly team meetings and individual reflective supervision
- Maintain strict confidentiality and exercise sound professional judgment at all times
Requirements
- Minimum of a high school diploma or equivalent (Bachelor's degree preferred)
- Experience working with or providing services to children and families
- Ability to establish trusting relationships
- Acceptance of and respect for individual differences
- Experience and humility to work with culturally diverse families
- Knowledge of infant and child development
- Infant mental health endorsement (preferred)
- Bilingual English/Spanish (required)
