About the Role

The Family Support Preventive Program provides support for families facing challenges that can impact their ability to parent their children and keep them safe. This program uses the Solution-Based Casework model to help families identify strengths and normative behaviors to address challenges. The Family Support Case Planner works with families experiencing circumstances that threaten the safety and well-being of their children, supporting and strengthening healthy growth and development within the family.

Responsibilities

  • Visit families in their homes, communities, or at the agency as dictated by the family’s circumstance and preventive mandates.
  • Engage the family using the Solution-Based Casework Model.
  • Document all case activity in the systems of record – N.Y.S. Connections and N.Y.C. PROMIS.
  • Advocate for clients in securing services such as public assistance, health insurance, and immigration support.
  • Coordinate and maintain contact with workers at the Administration for Children’s Services.
  • Maintain contact with service providers (e.g., schools, mental health facilities, therapists, substance abuse programs).
  • Participate in Family Team Conferences, Case Conferences, staff meetings, in-service trainings, and professional development opportunities.
  • Assist in emergency situations as needed.
  • Ensure compliance with all agency policies, procedures, and governmental regulations.
  • Perform other duties as required.

Requirements

  • Bachelor’s or Master’s degree in Social Work or a related field from an accredited college or university.
  • At least 1 year of experience in case management, counseling, and advocacy services (internship experience may count).
  • Strong interpersonal, organizational, written, and verbal communication skills.
  • Ability to accommodate a flexible work schedule.
  • Bilingual skills are a plus.

Pay

  • Bachelor’s degree: $27.47/hr – $28.57/hr
  • Bachelor’s degree (Bilingual): $28.57/hr – $29.67/hr
  • Master’s degree: $28.02/hr – $31.59/hr
  • Master’s degree (Bilingual): $29.12/hr – $32.69/hr
  • Master’s degree (Bilingual & Licensed): $31.32/hr – $35.43/hr
