Essential Duties and Responsibilities
- Provide direct health and social services to children & families
- Follow established program area timelines
- Conduct enrollment procedures and identify and address immediate needs
- Gather child health and development information and conduct screenings
- Establish trusting relationships
- Communicate effectively and assess individual family strengths and needs
- Assist families in setting and achieving goals
- Promote, organize, and facilitate educational trainings and meetings
- Collaboratively develop service plans
- Monitor individual case requirements, compile data, referrals, and follow-ups
- Regularly discuss status with Center Management Team
- Establish and maintain working relationships with community partners
- Facilitate advocacy and referrals for needed health, nutrition, and social services
- Develop, prepare, and maintain family records, documents, files, reports, databases
- Perform a variety of hours and be able to travel for work and training purposes
- Maintain standards of confidentiality of CC Idaho clients and records
- Perform all work duties and activities in compliance with CC Idaho, state, federal, and Head Start Performance standards and regulations
