FAMILY SERVICES ADVOCATE (MSHS - Seasonal Full Time) Osgood

Essential Duties and Responsibilities

• Provide direct health and social services to children & families
• Follow established program area timelines
• Conduct enrollment procedures and identify and address immediate needs
• Gather child health and development information and conduct screenings
• Establish trusting relationships
• Communicate effectively and assess individual family strengths and needs
• Assist families in setting and achieving goals
• Promote, organize, and facilitate educational trainings and meetings
• Collaboratively develop service plans
• Monitor individual case requirements, compile data, referrals, and follow-ups
• Regularly discuss status with Center Management Team
• Establish and maintain working relationships with community partners
• Facilitate advocacy and referrals for needed health, nutrition, and social services
• Develop, prepare, and maintain family records, documents, files, reports, databases
• Perform a variety of hours and be able to travel for work and training purposes
• Maintain standards of confidentiality of CC Idaho clients and records
• Perform all work duties and activities in compliance with CC Idaho, state, federal, and Head Start Performance standards and regulations
