Social Worker/Family Advocate

Job Responsibilities

• Provide support focused on the individual families' needs in the areas of health, education, nutrition and social services.
• Keep accurate documentation and maintain up-to-date records.
• Show attention to detail and possess strong organizational skills.
• Maintain caseloads with children ranging from 0-5 years as well as prenatal mothers.
• Create a positive and receptive learning environment for children and families.
• Develop individualized support plans based on the family and child's/children's needs.
• Record and document home visits and socialization attendance.
• Advocate for children and families and provide guidance and goal setting support for family success.
• Provide ongoing support and crisis interventions through direct services as needed.

Education/Position Requirements

• Bachelor's degree preferred, will consider Associates Degree.
• Must hold a valid Maine Driver's License with reliable transportation.
• Case Management experience preferred.

Salary

$25.00 - $28.00 per hour
