Responsibilities
- Complete screening and initial assessment interviews and enter initial data into the Program Management System for referred families.
- Enter assessments into the Danville-Pittsylvania Community Services and Healthy Families America/Virginia approved Medical Health Record database.
- Assist with and provide presentations to referring agencies within the community.
- Maintain and complete all required documentation as prescribed by Healthy Families America standards, licensure, and Medicaid regarding clinical services and medical records keeping.
- Comply with all regulations applicable to program services.
- Provide intensive home-based case management services to children at risk of serious emotional disturbance and their families to promote positive mental health development, family functioning, parent-child interaction, ensure receipt of appropriate medical care, and prevent child abuse.
- Develop and implement individual service plans.
Requirements
- Bachelor's Degree with at least 15 hours of coursework in human services and at least two years of experience working with children five and under, or a Bachelor's degree in Psychology, Social Work, Vocational Rehabilitation, Counseling, Sociology, Criminal Justice, or other approved Human Service degree, or Certified Psychiatric Rehabilitation Practitioner in VA.
- Valid driver's license and safe driving record sufficient to meet the requirements outlined in the Danville-Pittsylvania Community Services Policy Manual, Vehicle Usage Policy, and Procedures.
Pay
Annual salary range: $44,825 - $77,617
