Position Overview

Provides for the safety, stability and well-being of children and families during the referral period
Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models
Mets DCF timelines to connect with the family within 24 hours of referral; facilitates initial family meeting within 48 hours of referral; completes assessment-based case plan within 20 days of referral; and case closure within 3-6 months of referral

Responsibilities

  • Engages and works in partnership with families to complete assessments and develop case plan goals and objectives
  • Mets with the family in the home weekly monitoring the family's progress towards their case plan goals; and meets monthly with each child individually to assess child's safety
  • Assesses each child's safety with every family contact and reports all suspected child abuse / neglect
  • Monitors family progress and service provision using assessment tools, and updates family case plan based on those assessments
  • Provides weekly home-based family skills training which may include: Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication to assist parents in providing a safe, nurturing environment for children in their care
  • Works closely with the Family Support Worker to provide services for the family and make decisions about child well-being

Requirements

  • Bachelor's degree in human services required
  • Minimum two years relevant experience
  • Unrestricted Class C driver's license, a Motor Vehicle Record (MVR) that meets DCCCA's insurability standard, reliable personal transportation, and in force automobile liability insurance, required

Qualifications

Must successfully complete pre-employment background screening, references, fingerprint-based records search, and pre-employment drug screening.

Family Preservation Services (FPS)

Provides intensive in-home services and case management services to families who have one or more children at risk of out-of-home placement or who will be at risk of out-of-home placement at birth.

Qualified candidates are eligible for a $1,000 sign-on bonus (half at six months, half at one year anniversary)
