Family Preservation Case Manager
About the role
Clarvida is seeking a Family Preservation Case Manager to work directly with children involved with DCFS and their families to promote safety, stability, and long-term success.
Responsibilities
- Meet with children and families involved with DCFS to assess needs and strengths
- Develop individualized goals focused on safety, stability, and family preservation
- Link families to appropriate services, resources, and treatment supports
- Conduct regular home and community-based visits
- Maintain accurate, timely, and compliant case documentation
- Collaborate with supervisors, internal teams, and external partners
- Participate in supervision, team meetings, and required trainings
Requirements
- One of the following:
- A Bachelor’s Degree in a Human Services field (Social Work, Psychology, Counseling, or related field) with a minimum of two (2) years of related experience
- A Master’s Degree in a Human Services field (Social Work, Psychology, Counseling, or related field)
- CWEL license preferred
- Valid Driver’s License in good standing and active automobile insurance
- Ability to obtain an Illinois Driver’s License within thirty (30) days of hire if relocating from out of state
Compensation & Benefits
- Annual salary of $50,000
