About the Role
The provider will manage a diverse patient panel, conducting acute, chronic, and wellness visits in-office, via testing centers, or through telehealth. The role requires obtaining or maintaining state licensure to provide virtual primary care across multiple states.
Responsibilities
- Managing patient needs across various visit types (acute, chronic, wellness)
- Providing care in-office, at testing centers, and via telehealth
- Collaborating with clinical teams and participating in ongoing education (e.g., weekly rounds)
- Performing in-office procedures as applicable
- Utilizing clinical training for procedures and evidence-based primary care
Requirements
- Completion of an accredited FNP or PA program with national certification
- At least 2 years of outpatient primary care experience for recent practitioners, or 1 year plus additional fellowship or urgent care experience
- State licensure in Connecticut or relevant state upon start
- Strong communication skills and clinical proficiency in evidence-based primary care
- Comfort with technology for telehealth and virtual care
