About the role

Seeking a full-time primary care provider responsible for managing a diverse patient panel, including acute, chronic, and preventive visits. Duties include in-office and telehealth consultations, performing in-office procedures, and collaborating with both in-person and virtual clinical teams. Commitment to continuous learning through weekly clinical rounds and other modalities is essential. The role may require obtaining additional state licensure for virtual care in other states.

Responsibilities

Qualifications

• Completion of an accredited FNP or PA program with national certification
• At least 2 years (or 1 year with additional fellowship or urgent care experience) of outpatient primary care practice seeing all ages within the past 5 years
• Current state licensure in Connecticut
