Job Overview
Family Nurse Practitioner will provide primary care services for adults, according to certification, in federal-qualified health center settings to diverse and underserved populations. Reports To: Medical Director. Affiliation: SHS (PHMC Health). Status: Salary Exempt.
Essential Duties and Responsibilities
- Provide primary care services.
- Work within all state and federal laws and regulations and the agreement with the collaborative physician.
- Manage acute and chronic health problems using evidence-based protocols.
- Provide health screenings, immunizations, and other disease prevention programs.
- Provide age-appropriate assessment and interventions to meet the needs of patients and/or families served.
- Provides patient/family education, health counseling, and health advocacy services to patients and/or their families.
- Be willing to work occasional weekends and on-call.
- Collaborate with clinical staff on care and follow-up.
- Participate, as needed, in public health management of communicable diseases in shelters and in other health initiatives.
- Maintain positive relationships with funding sources and community organizations.
- Perform other related tasks and duties as directed and assigned.
- Work as a member of the Primary Care Medical Home team to provide integrated health services.
- Document all services in a timely manner.
- Act in the capacity of a mandated reporter to both the State of Pennsylvania and the Joint Commission.
PHMC Compliance Responsibilities
- Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers and Conflict of Interest Policies.
- Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
- Complies with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
- Complies with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards.
- Adheres to all PHMC Policies and Procedures.
- Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures.
Job Requirements
- Credential Required: Licensed as a Registered Nurse and APN Nurse Practitioner.
- Primary Source Verification: PA Division of Consumer Affairs, Board of Nursing.
- Mets Credentialing Requirements as stipulated by Medicare, Medicaid, and private insurers.
- Preferred credentialed with insurance companies.
- Skills: Ability to collect adequate client data, conduct a physical exam, interpret physical findings and manage or refer patients as conditions or protocols dictate.
- Good verbal communication and writing skills.
- Computer skills.
- Ability to communicate and work with variety of patient populations, some with complex needs.
- Ability to prioritize and organize workday to meet changing needs of program and clients.
- Ability to work within a team framework and respond positively to team members concerns.
- Ability to track work to completion and compile reports.
- Experience: Three (3) years of nursing experience, at least two (2) of which shall have been as an Advanced Practice Nurse in a related specialty, preferred, but not required.
- Education: Licensed as a RN and FPN Nurse Practitioner.
- Certified as an Advanced Practice Nurse in the State of Pennsylvania.
