Family Nurse Practitioner

About the role

The Family Nurse Practitioner will provide evidence-based Primary Care to patients at the South Bronx Health Center. They will be part of a collaborative care team that includes nursing staff and other services like Health Education, Nutrition, Social Work, and Behavioral Health.

Responsibilities

• Provide primary care services to patients in a Federally Qualified Health Center setting
• Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care
• Adhere to evidence-based practices and guidelines in delivering care
• Conduct health assessments, diagnose conditions, and develop treatment plans
• Participate in interdisciplinary team meetings to discuss patient cases and plan care
• Document patient interactions and care plans accurately and timely

Requirements

• A valid Family Nurse Practitioner license from the State of New York
• At least two (2) years of post-graduate/residency experience in a Primary Care/Ambulatory Care setting

Qualifications

• Strong clinical skills and knowledge of primary care medicine
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Passion for working with underserved communities
