What You Will Do
- Deliver comprehensive primary care with a strong focus on prevention and continuity
- Serve as a clinical leader and trusted partner to patients and families
- Collaborate with pharmacists, care coordinators, and virtual care partners
- Help design workflows, care standards, and patient experience from the ground up
- Contribute ideas and leadership as the clinic scales and evolves
Who You Are
- A Family Nurse Practitioner with a Master’s or Doctoral degree in Nursing
- Licensed in Pennsylvania with national certification such as FNP-BC or FNP-C
- Experienced in primary care, community health, or rural health environments
- Confident using EHR systems and telehealth platforms
- Entrepreneurial, adaptable, and excited by building something meaningful
- Deeply committed to patient-centered, relationship-driven care
