About the role
We are seeking a Full-Time Family Nurse Practitioner for our South Bronx Health Center located at 81 W. 115th Street, New York, NY 10026. Reporting to the Medical Director, this individual will provide evidence-based primary care to patients as an integral part of a collaborative care team, which includes nursing staff, health education, nutrition, social work, and behavioral health.
Responsibilities
- Deliver evidence-based primary care to patients in an ambulatory setting.
- Collaborate with a multidisciplinary care team, including nursing staff, health educators, nutritionists, social workers, and behavioral health professionals.
Requirements
- Valid Family Nurse Practitioner N.Y.S. License.
- At least two (2) years of post-graduate/residency experience delivering care in a primary care or ambulatory care setting.
