Summary/Primary Purpose

Essential Duties And Responsibilities

  • Conduct orientation for new families including completion of Parent Handbook forms and required documentation.
  • Teach child development and parenting skills to increase family self-sufficiency and school readiness utilizing the researched-based curriculum Parents As Teachers (PAT).
  • On a weekly basis, complete 90min. home visits with assigned families, including child development observations, to ensure a minimum of 46 visits per family per program year is completed. Makeup missed home visits as needed to meet minimum requirements.
  • Document home visits in the ChildPlus Home Visit Module, Family Services Event, and ChildPlus DRDP within 24hrs of completion.
  • Every Monday, plan for weekly home visits including PAT parent handouts, child development activities, and child observations ensuring all ChildPlus DRDP measures are completed.
  • Conduct Brigance screenings and ongoing assessments according to program policy guidelines and monitor child development progress on an ongoing basis.
  • Integrate information from assessments, parents, and, if applicable, the Individual Family Service Plan (IFSP), to identify child development goals utilizing the EHS Ongoing Child Development Assessment Chart. Document progress of child goals each month in ChildPlus.
  • Complete Family Outcomes Assessments for each family and develop Family Partnership Goal Plans with parent input, including documentation of progress each month in ChildPlus.
  • Conduct a Prenatal Risk Assessment with pregnant women, and with input from the family and Health Services, develop an individualized Prenatal Service Plan. Teach expectant families prenatal topics required by the Federal Performance Standards utilizing the researched-based curriculum, Partners for a Healthy Baby: Before Baby Arrives. Assist families in the completion of monthly health requirements, based on Nebraska’s EPSDT schedule and program requirements, to meet the individual healthcare needs of enrolled children and pregnant women. Teach families at least one monthly health topic provided by Health Services every month and document completion on the Home Visit Plan.
  • Ensure new families meet with the Mental Health Consultant within 90 days of enrollment, annually and 6wks postpartum. Provide each family with referrals to community agencies or services to meet their individualized needs. Ensure Weekly Plans are completed by each family and enter In-Kind transactions in ChildPlus on a weekly basis.
  • Complete 30mo. Progress Reports for children at 30mo. of age, Final Transition Plans for children graduating from the program, and Discharge Summaries for all closing families. Maintain high quality documentation in ChildPlus family files and ChildPlus DRDP within 24hrs of completion.
  • Attend quarterly Multi-Disciplinary Team meetings and present on each assigned family.
  • Attend and participate in all program-required socializations, events, meetings and trainings.
  • Follow State mandatory reporting laws, including program policy, in cases of child neglect/abuse.
  • Driving is an essential aspect of this position. If bilingual, perform interpretation, translate documents and help families' complete forms.

Education and/or Experience

  • Associate degree (A.A.) in Early Childhood Education, Social Work, Human Social Services or related field and at least one-year related experience required.
  • Bachelor’s Degree (B.S. or B.A.) in same fields may be given preference.
  • If hired as Bilingual Family Consultant, ability to speak, read and write in language of origin is required.

Qualifications

  • Bilingual Spanish, Karen or Karenni strongly preferred.
  • Ability to be flexible and able to work on multiple projects or tasks simultaneously.
  • Administrative skills to include utilization of standard office equipment, Microsoft Office and Excel, data entry, attention to detail and filing.
  • Good communication skills both written and spoken, and ability to maintain effective working relationships.
  • Demonstrate sensitivity to the service population’s cultural and socioeconomic characteristics.

Other Qualifications

  • Must pass all applicable background checks.
  • Must be able to pass medical examination and Tuberculosis test.
  • Must possess a valid driver's license from the state in which you reside.
  • Must be approved through The Salvation Army Fleet Safety Program to drive either a Salvation Army or personal vehicle on Salvation Army business.
  • Must be able to provide own transportation to home visits.

Certificates, Licenses, Registrations

  • Obtain certification in First Aid, CPR and CPI within 6 months of hire.
  • Become certified in Parents As Teachers within 6mo of hire.
  • Become certified in Partners for a Healthy Baby curriculum within two years of hire.

Physical Requirements

  • While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle, or feel; reach with hands and arms.
  • The employee is frequently required to stand, walk, and talk or hear.
  • The employee is occasionally required to stoop, kneel, crouch, or crawl and climb or balance.
  • The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.

Working Conditions

  • While performing the duties of this job, the employee is regularly exposed to outside weather conditions during home visits or outings with families.
  • The noise level in the work environment is usually moderate.
