Family Centered Specialist

About the role

Mentor Maryland, a part of the Sevita family, provides Residential and Treatment Foster Care services to youth facing various out-of-home situations. Our mission is to help youth gain stability, comfort, and confidence to overcome challenges and thrive.

Responsibilities

• Provide assessments of individuals and families in areas like mental status, personality, behavioral patterns, relationships, and family interactions.
• Develop and implement service or treatment plans based on the assessment of family needs.
• Offer 24/7 coverage for assigned individuals and families.
• Create and maintain closure and aftercare plans for each family and individual served.
• Build and maintain positive relationships with personnel from referring and collateral agencies, guiding families in their interactions with these agencies and other community organizations as needed.
• Provide referrals to services and resources on behalf of families.
• Ensure confidentiality, organization, and completeness of documentation and records, submitting them accurately and promptly to supervisors.
• Travel up to 1 hour for families served.
• Participate in team meetings with peers and collateral team meetings as needed.
• Support peer development by addressing professional development and team performance issues, providing training or field supervision, reviewing and editing peer records and documentation, and participating in marketing and education presentations.

Qualifications

• Bachelor's Degree in a related Human Service field required.
• One year of case management or behavioral health experience.
• Specialized training required.
• Self-motivated and detail-oriented with the ability to multi-task.
• Excellent communication and analytical skills.
• Valid Driver’s license and reliable vehicle, including valid registration and insurance.
