Family Care Manager (Licensed Social Worker) - Ohana Higher Level of Care
About the role
The Licensed Family Care Manager supports children and adolescent patients and their families presenting to Ohana with complex psychiatric and social problems. They provide guidance, support, and facilitate referrals to external services.
Responsibilities
- Supports patients and families in understanding and managing various issues such as legal, educational, financial, occupational, economic, and medical matters.
- Facilitates referrals to external services and programs when necessary.
- Engages in ongoing efforts to learn about and build collaborative relationships with local and regional resources for the families of Ohana patients.
- Assists with psychoeducation and workshops as needed to support the Ohana clinic.
- Attends treatment team meetings and clinical rounds, working closely with therapists, psychiatrists, and other behavioral health staff.
- Supervises interns when needed.
- Fulfills other necessary tasks as assigned by the leadership team.
Requirements
- A minimum of 3 years' experience working with children, adolescents, or families.
- Knowledge of local and regional resources available to Ohana's patient population.
- Care Coordination experience is preferred.
- A Master's degree in Social Work is required.
- Licensure with the California Board of Behavioral Sciences as an LCSW or LMFT is required.
- American Heart Association Healthcare Provider BLS certification is required.
- Completion of organization-provided AVADE training.
- Completion of mandatory 5150 training by Monterey County to obtain 5150 certification.
- Live Scan fingerprint clearance is required by the Department of Healthcare Services.

The pay range for this position is $52.26 to $69.89 based on years of applicable experience.
