Faith Community Nrs Coord

Responsibilities

• Serves as a liaison between the healthcare system, local faith communities, and the community at large.
• Guides both paid and volunteer FCNs in providing holistic care that integrates physical, emotional, and spiritual well-being.
• Recruits and orientates FCNs.
• Affirms health needs and coordinates and performs wellness screenings and education events, as well as presentations to nurses and clergy.
• Maintains program data and ensures compliance of data collection from FCN Volunteers and self-reporting on Community Benefit Form.
• Fosters partnerships with congregations and community organizations to align services with BayCare’s health priorities and Community Health Needs Assessments (CHNAs).
• Carefully coordinates care of the clients enrolled in the FCN Transitional Care Program.
• Responsible for transportation between facilities and to community events.

Requirements

• Education: Associate's degree in Nursing or Diploma Nursing.
• Preferred Education: BSN (Bachelor of Science in Nursing).
• Required Certification: BLS (Basic Life Support) and RN (Registered Nurse).
• Required Experience: Minimum 3 years of direct patient care experience.
• Working knowledge and computer proficiency (Microsoft suite).
