The Facilitator ensures the principles and phases of the High-Fidelity Wraparound process are delivered with fidelity to the model, supporting families in achieving positive outcomes.
Responsibilities
- Engage youth and families in care planning and monitoring.
- Develop integrated plans and natural support systems.
- Maintain strong relationships with families and community partners.
- Ensure fidelity to High-Fidelity Wraparound principles.
- Document all activities accurately and timely.
Requirements
Bachelor’s degree in human services or related field (experience may substitute). Two years of experience providing services for children, youth, and families.
