Education and Experience

• Bachelor’s degree from a four-year college or university in Exercise Science/Physiology/Kinesiology
• Prior experience working with a frail or elderly population preferred

Certificates, Licenses, Registrations

• Valid Florida drivers license
• Current auto insurance
• Reliable transportation
• CPR certification from the American Heart Association or Red Cross
• Alzheimer's Disease and Related Dementias Training from the Florida Department of Elder Affairs

Mathematical Skills

• Add, subtract, multiply, and divide in all units of measure
• Compute rates, ratios, and percentages
• Draw and interpret bar graphs

Reasoning Ability

• Solve practical problems
• Deal with a variety of concrete variables
• Interpret written, oral, and visual information

Language Skills

• Read and interpret documents such as safety rules, operations, and maintenance instructions
• Write routine reports and correspondence
• Speak effectively before groups
• Read and understand general business periodicals, professional journals, and governmental regulations

Position Requirements (not All Inclusive)

• Review participant’s medical history
• Determine participant’s strengths and limitations
• Develop and schedule individualized or group exercise programs
• Teach participants how to perform exercises safely and effectively
• Measure and monitor participants’ vital signs
• Educate participants about their exercise program
• Collaborate with clinical team/PT/OT
• Complete documentation in PACE EMR
• Use of electronic DME order system
• Maintain supplies and equipment in clean/orderly and good working condition
• Maintain confidentiality of participant information (HIPAA)

Compensation

The pay range for this position is $22.30 - $33.18 per hour. A conditional offer of employment will require consent to drug and/or alcohol testing, as well as ongoing compliance with the Drug-Free Workplace Policy. All Chapters Health System employees performing services for Florida affiliates must undergo background screening through the Florida Care Provider Background Screening Clearinghouse after a conditional offer of employment is made, and ongoing eligibility is verified.
