Principal Responsibilities
- Congduct group and individual outreach to explain JWCH's clinical services and programs.
- Perform outreach to educate individuals about health care options such as Medi-Cal, Healthy Families, Healthy Kids, Healthy Way LA, and others in diverse settings like Clinic sites, head starts, schools, churches, CBOs, homeless shelters, and health fairs.
- Provide comprehensive assistance to children and their families, including in-reach and outreach to target members and support during the application process.
- Follow up to verify enrollment and encourage the utilization of health insurance benefits and services.
- Offer assistance to members facing challenges during enrollment, advocating on behalf of families, and addressing barriers to enrollment, utilization, and retention.
- Support families in enrolling for health care services and help them access and retain these services effectively.
- Ensure proper enrollment of prenatal patients and their newborns into Medi-Cal, Healthy Families, and other health care options.
- Assist CHDP - Gateway patients in accessing all available services.
- Conduct presentations about health care options like Medi-Cal, Healthy Kids, Healthy Families, and Healthy Way LA to community members and other CBOs.
- Engage in outreach activities at health fairs and other gatherings.
- Maintain records, logs, signing sheets, and necessary documentation to track compliance and evaluate program performance.
- Plan and conduct outreach activities in the field, as well as orientation meetings, trainings, workshops, and informational events for childcare providers, schools, and community-based programs, shelters, churches, and other venues.
- Be available for evening and weekend work as assigned.
- Provide assistance to the supervisor with special tasks or events, including trainings, workshops, special mailings, and material reproduction.
- Attend and actively participate in external meetings and events related to scope of work.
- Prepare program reports according to departmental policy and procedures.
Requirements
- Minimum of one year of experience in health care outreach and enrollment.
- Bilingual- Fluent in both English and Spanish.
- Possess a current valid California Driver License and Insurance Certificate.
- Excellent oral and written communication skills.
- Must possess a caring and compassionate demeanor.
- High School Diploma or equivalent required.
- Proficiency in working with diverse socio-economic, ethnic, and cultural backgrounds among clients, patients, and staff.
- Certified Application Assistant (CAA) qualification is preferred.
- AA Degree in a related field (Public Health, Health-Care Administration, Human Services) preferred.
